1500 link to file a claim online. Remember when filing a
claim, a copy of the Tricare EOB is necessary to pay your claim so
please write your certificate # on the EOB and fax it to 301-816-1125 or
mail to P.O. Box 2510, Rockville, MD 20847-2510.
Checking Eligibility is also a snap. Click on the Eligibility
link to check the status of an insured.
